Tsartlip Election – Request for Mail-in Ballot
Please find here the form to request a mail-in ballot for the upcoming election for Chief & Council being held December 06, 2021.
Please download, print and complete this form and return it to the Electoral Officer by mail or e-mail. by November 29, 2021.
You must also include a copy of proof of identity, such as a copy of your Certificate of Indian Status, driver’s license, health card, or another document that proves your identify.
Do not provide the original identification.
If the Electoral Officer receives this request on or before November 29, 2021, a mail-in ballot package will be sent to you at the address provided. If you have any questions, please contact the Electoral Officer.
